CRT-D Therapy in Patients with Decompensated NYHA Class-Four CHF

چکیده
Background. ACC-HRS Guidelines for Cardiac Resynchronization Therapy ICD implantation (CRT-D) do not include patients with advanced nonambulatory NYHA class-four CHF due to an expectation of limited survival. There is little data available from these large multicenter randomized studies to support or refute this claim. Purpose. We evaluated the outcomes of patients with advanced nonambulatory NYHA class-four CHF who received CRT-D devices as an attempt to improve the clinical status and promote hospital discharge. Methods. Sixteen (of our six hundred and seventy CRT-D patients) were classified as advanced nonambulatory NYHA Class four inotrope/vasodilator/diuretic-dependent patients. These patients were analyzed retrospectively for weaning success to oral medications, hospital discharge, hemodynamic stability, and survival over eighteen months. Results. Thirteen of sixteen patients were discharged to home within two weeks of implantation. The survival to hospital discharge, as well as at six, twelve, and eighteen months was positive (ninety-four percent, seventy-five percent, sixty-nine percent, sixty-nine percent, resp.). The groups showed significant improvements in systolic blood pressure, renal function, left ventricular ejection fraction, and CHF class. Conclusion. CRT-D in advanced nonambulatory NYHA four patients proved feasible and beneficial. These findings suggest that the strategy merits further study.

When is it too late for cardiac resynchronization therapy?
Lehmann et al.1 pointed out that taken together, the large cardiac resynchronization therapy (CRT) trials included 10,803 patients, the majority of who were in congestive heart failure (CHF) New York Heart Association (NYHA) class III, but only 451 patients (4.2%) were class IV. The role of CRT in NYHA class IV patients remained uncertain until recently when Lindenfeld et al.2 reported the resu...
متن کاملCardiac Resynchroniztion Therapy In Heart Failure: Recent Advances And New Insights
Newer non-pharmacological therapies for heart failure are being evaluated for patients of congestive heart failure (CHF). Mechanical support with left ventricular assist devices and heart transplantation are reserved for the minority of patients who have severely decompensated heart failure. Despite these therapeutic advances, it is generally accepted that current therapies do not adequately ad...
متن کاملDifferent response rates to cardiac resynchronization therapy (CRT) according to the applied definition.
BACKGROUND Solid evidence shows that cardiac resynchronization therapy (CRT) improves prognosis, physical capacity and quality of life in selected groups of patients with chronic heart failure (CHF). Nonetheless, marked percentage of patients seem not to benefit from CRT. MATERIAL AND METHODS Sixty consecutive patients (aged 66.3 +/- 8.7 years, 57 men - 95%, 3 women - 5%) with CHF (71.7% with...
متن کاملEffects of cardiac resynchronization therapy with or without a defibrillator on survival and hospitalizations in patients with New York Heart Association class IV heart failure.
BACKGROUND Cardiac resynchronization therapy (CRT) alone or combined with an implantable defibrillator (CRT-D) has been shown to improve exercise capacity and quality of life and to reduce heart failure (HF) hospitalizations and mortality in patients with New York Heart Association (NYHA) class III and IV HF. There is concern that the device procedure may destabilize these very ill class IV pat...
متن کاملDronedarone in patients with congestive heart failure: insights from ATHENA
AIMS Dronedarone is a new multichannel blocking antiarrhythmic drug for treatment of atrial fibrillation (AF). In patients with recently decompensated congestive heart failure (CHF) and depressed LV function, the drug was associated with excess mortality compared with a placebo group. The present study aimed to analyse in detail the effects of dronedarone on mortality and morbidity in AF patien...
